The Vision PPO plan provided by Anthem gives you the freedom to see the vision provider of your choice while offering cost protection through a network of eye care providers.
When you utilize an in-network eye care provider, your out-of-pocket expenses will be less.

In-Network
- Vision Exam | $10 Copay Is this sentence blurry? Get your eyes checked! You’re covered for one exam every 12 months. You’ll pay $10 for a basic exam as long as you stay in-network.
- Frames | Up to $150 Allowance, then 20% off any remaining balance Choose from a selection of standard frame brands and receive a $150 allowance if you stay in-network. If you purchase frames that are over $150, you get 20% off the difference in price.
- Single Vision, Lined Bi/Trifocal Lenses | $25 Copay
- Contact Lenses (Medically Necessary) | Covered-in-Full
- Contact Lenses (Elective) | Up to $150 Allowance, then 15% off any remaining balance

Out-Of-Network
- Vision Exam | Up to $42 Reimbursement Is this sentence blurry? Get your eyes checked! You’re covered for one exam every 12 months. You’ll will get reimbursed up to $42 for a basic exam when going out-of-network.
- Frames | Up to $45 Reimbursement Out-of-Network you will receive a reimbursement of up to $45 for frames.
- Single Vision, Lined Bi/Trifocal Lenses | Up to $40-$80 Reimbursement After your copay, going out-of-network you will receive a reimbursement of up to $80 for glasses or plastic single vision, lined bifocal, and lined trifocal lenses.
- Contact Lenses (Medically Necessary) | Up to $210 Reimbursement Going out-of-network, after your copay, you will receive a reimbursement of up to $210 for contact lenses.
- Contact Lenses | Up to $105 Reimbursement Going out-of-network, after your copay, you will receive a reimbursement of up to $105 for contact lenses.
