  • Deductible | $50 Single ($150 Family) You are responsible for the full cost of your care until you hit this amount during your plan year.
  • Annual Maximum | $1,500 Per Member This is the most your plan will spend on dental expenses per person, per plan year.
  • Preventive Care | 100% In-Network (80% Out-of Network) Diagnostic & Preventive Care includes cleanings, exams, x-rays, sealants, and other services.
  • Basic Services | 80% In-Network (70% Out-of Network) Basic Care includes fillings, root canals, periodontics, and other services.
  • Major Services | 50% In-Network (50% Out-of Network) Major Care includes crowns, inlays, onlays, restorations, bridges, dentures, and other services.
  • Orthodontics (Adult & Children) | 50% Max - $1,500 Lifetime Benefit Need your teeth straightened? You’re covered. Keep in mind, there is a $1,500 lifetime maximum for orthodontic services per person for in-network and out-of-network.
