HSP offers employees a Medical HMO Plan. This network of providers requires you to elect a Primary Care Physician to help direct your care. They will direct care if you ever have urgent care, specialist or hospitalization needs. They also provide great service tools to help you navigate the healthcare marketplace.

  • Deductible | $3,000 Per Person The amount you owe for health care services before your health insurance or plan begins to pay.
  • Maximum Out Of Pocket | $6,400 Individual ($12,800 Family) The money you pay for covered medical and pharmacy services counts toward your out of pocket maximum. Once you meet your designated out of pocket maximum, the plan will pay for all of your covered healthcare costs for the rest of the plan year.
  • Primary Care Doctor | $30 copay A PCP is the person you and /or your dependents should see for a routine checkup or non-emergency medical care
  • Specialist | $50 copay A physician specialist focuses on a specific area of medicine or a group of patients to diagnose, manage, prevent or treat a certain types of symptoms and conditions
  • LiveHealth Online | $0 copay PCP / $50 copay Specialist Telemedicine refers to the practice of caring for patients remotely when the provider and patient are not physically present with each other. Modern technology has enabled doctors to consult patients by using HIPAA compliant video-conferencing tools.
  • Urgent Care | $30 copay Care for an illness, injury or condition serious enough that a reasonable person would seek care right away, but not so severe as to require emergency room care.
  • Hospital | 30% after deductible A center where you go when you need care or surgery. You may go to a hospital, get your treatment and then go home that day (outpatient). Or your condition or the care you need may require you to stay over for one or more nights (inpatient).
  • X-Ray & Labs | No Charge Tests and procedures ordered by a doctor. May include radiology, ultrasound, nuclear medicine, laboratory, pathology and others.
  • Emergency Room Visit | $250 copay and 30% after deductible The Emergency Room (ER) provides care for these critical or life-threatening conditions and not for routine health care.
